An ongoing inquiry is examining the circumstances surrounding a fatal incident involving Rudakubana, who was 17 at the time. The inquiry was informed that a risk assessment was conducted for Rudakubana one week prior to the July incident last year, coinciding with his brother, Dion Rudakubana, publicly raising questions about whether the attack could have been prevented.
Details presented to the inquiry reveal that Rudakubana and his family had been receiving support from child and adolescent mental health services (Camhs) since February 2021, when he was diagnosed with autism. Mental health professionals carried out a final risk assessment on July 23 last year, six days before the incident, leading to his formal discharge from Camhs.
Specialists involved with Camhs reportedly expressed concerns regarding Rudakubana’s parents, whose role was described as “consistently unhelpful” in his engagement with mental health services. A Lancashire county council witness statement further noted that in August 2021, after Rudakubana turned 15, Camhs documented his disclosure of “physical aggression and emotional abuse at home.” A social worker also reported “tension with the father” and an incident where Rudakubana poured milk on his parents’ bed during a family meeting that same month. The statement clarified that no recent violence between Rudakubana and his father had been reported.
